What do I need to bring the day of my procedure?
- Any change of address or phone update.
- Your insurance card(s).
- A picture ID.
- A current list of all medications and dosages.
- A current list of any vitamins, herbs or supplements you’re currently taking.
- A list of any known allergies.
Please leave any jewelry or valuables at home.
How do I schedule or cancel an appointment?
To schedule or cancel a procedure, please call your physician’s office. You should also contact your physician’s office to follow up on any results or if you have any urgent after hours concerns.
